Jo (Noémie Merlant) is a successful, highly curated lifestyle entrepreneur and influencer whose pregnancy and impending motherhood are greeted with picture-perfect enthusiasm by her online followers. Alongside her supportive husband Spencer (Kit Harington), she prepares for the arrival of her daughter, Ruby, believing she can balance career, domestic life, and motherhood effortlessly.
However, following a painful birth, Jo's pristine reality crumbles. Baby Ruby cries endlessly and exhibits an unsettling hostility exclusively toward Jo. As sleep deprivation and severe isolation take hold, Jo suffers from harrowing hallucinations, violent paranoia, and rising hostility from her husband and mother-in-law, who dismiss her distress as simple exhaustion—leaving Jo to question whether her baby is truly malicious or if she is losing her mind to postpartum psychosis.
